Through our multi-disciplinary team approach, we teach patients the skills they need to confidently manage their condition, maintain their independence, and get back to doing what they love. An important part of patient-centered care is giving people the flexibility to be treated where they are most comfortable. We are undergoing a significant expansion of our in-home care footprint and capabilities, to better serve our seniors who face transportation or mobility challenges—or who just prefer having their appointments in the safety and convenience of their homes. In-home care helps patients feel at ease and promotes long-lasting and personal provider relationships.
